- State issued birth certificate. Hospital copies are not accepted. The official Birth Certificate is required for the MYSA player card

Fees
Below is the list of current fees. Subject to change
- Club Fees: $300 / year
- $150 Due Feb 1
- $150 Due Aug 1
- Troy United Kits (uniforms): Currently $95 Ordered through the Club (includes player number and Troy United SC team logo on jerseys)
- We Proudly wear Puma!
- White jersey, white shorts and white socks
- Purple jersey, purple shorts and purple socks
- *Additional items such as back packs, warm ups, car decals, hoodies, t-shirts, etc., can be ordered through our store, see your team manager for current store items and prices. *

- League Fees
- SLYSA: $120-150 est. Spring 2013
- MYSA Seasonal Player Card, required by all players and coaches: $20 per season
